Health Department Announces Closure of Certified Home Healthcare Agency

OSWEGO – The Oswego County Health Department has announced it will no longer admit new patients to the Oswego County Certified Home Healthcare Agency starting immediately.

The New York State Department of Health authorized the Oswego County Health Department to no longer accept referrals for home health care services Oct. 19.

The County Health Department is following New York State guidelines, and has been working closely with local health care providers and contractors since receiving approval from the New York State Department of Health to close the Oswego County CHHA.

“Oswego County clinicians have met or will meet with all of the active Oswego County CHHA patients to ensure a smooth transition and to make certain there is no loss of services,” said Oswego County Public Health Director Jiancheng Huang. “The Health Department staff is available to assist patients and their families in making an informed choice to transfer to another one of the excellent licensed Certified Home Health Agencies which provide services in Oswego County.”

Oswego County has notified referring physicians of the closure and asked that, in the future, they refer all patients in need of home care services to the authorized home health agency of their choice.

New York State Department of Health approved CHHA providers serving Oswego County are Gentiva Health Services, Home Care of Rochester, Oswego Health Home Care, Inc., and VNA Central New York.

As the state-approved plan directs, the public will be informed through the Oswego County website at and through news releases distributed by  the Oswego County Department of Community Development, Tourism and Planning.
