OSWEGO COUNTY — On behalf of Oswego Hospital Extension Clinic, this is to notify the public that the Parish Health Services Center, located at 10 Carlton Drive, Parish, NY 13131, anticipates closing upon approval of the NYS Department of Health.
Physical therapy and outpatient services will continue at the Central Square Medical Center, which is approximately 9.59 miles from the current site, and all Parish staff will transition to that location to ensure uninterrupted care.
A town hall meeting will be held to discuss the closure and relocation plan on Tuesday, October 28, 2025, from 5:00 – 6:00 p.m. at the Parish Village Gym, 2938 East Main Street, Parish, NY 13131.
