Pulaski Health Center Encourages Enrollment At School-Based Health Center At APW High School

PULASKI, NY – Northern Oswego County Health Services, Inc. (NOCHSI), operator of the Pulaski Health Center, is encouraging families to enroll their high school students at its school-based health center at the Altmar Parish Williamstown High School in Parish.

NOCHSI opened the APW school-based health center in January.

It joined NOCHSI’s other school-based health center programs at Sandy Creek Central School, Pulaski Middle/Senior High School, and Lura Sharp Elementary School.

Enrollment packages were mailed to the families of APW high school students in mid-August.

The school-based health center staff will be available at freshman orientation on Aug. 26 and at an open house at the school scheduled for Sept. 25 to provide enrollment forms and offer assistance.

The school-based health center at APW provides primary and preventive health care service to students.

The center is staffed by Dr. Jay Chapman, Louise Mongelluzzo, a physician’s assistant, Justine Hammond, a behavioral health specialist, and a medical assistant.

Health care services will be provided throughout the school year during classroom hours.

There is no out of pocket expense for services rendered at the center.

The program is designed to complement the health care provided to students through their family physician.

Those students and families without a family physician are welcome to contact the Pulaski Health Center at 298-6564 to register with one of its physicians.

“Over the short period of time since the opening of the APW school-based health center, nearly 50% of students enrolled in the program and over 500 student visits were conducted.  NOCHSI is delighted with the initial success of the program and is looking to achieve over 90% enrollment in the program by the end of the year,” said Daniel T. Dey, executive director.

Located at 61 Delano St., the Pulaski Health Center is a family-oriented health care practice that provides Northern Oswego and Southern Jefferson County residents with a variety of comprehensive health care and related services.

The center is accredited through the Joint Commission on Accreditation of Healthcare Organizations.
