Oswego Health Home Care Recognized Among Nation’s Best for Patient Experience

OSWEGO – Oswego Health Home Care has been recognized by Strategic Healthcare Programs (SHP) as a “Superior Performer” for achieving an overall patient experience score that ranked in the top 20% of all eligible SHP clients for the 2025 calendar year.

The annual SHPBest™ award program was created to acknowledge home health agencies that consistently provide high quality service to their patients. The 2025 award recipients were determined by reviewing and ranking the overall experience score for more than 3,200 home health providers. With the largest HHCAHPS benchmark in the nation, SHP is in a unique position to identify and recognize organizations that have made patient experience a priority and have been rewarded for their efforts with high marks on the HHCAHPS survey.

For Oswego Health Home Care, the recognition reflects the dedication of a team that brings skilled, compassionate care directly to patients where they are most comfortable — at home. Oswego Health Home Care is the only hospital-owned certified home health agency in Oswego County, providing skilled nursing, rehabilitation and support services 24 hours a day, seven days a week. The team works closely with patients, families and providers to help individuals recover from illness or injury, manage chronic conditions and maintain as much independence as possible at home.

The 2025 recognition is another achievement for an experienced Home Care team that has consistently demonstrated a commitment to quality and patient satisfaction. Oswego Health Home Care was also recognized by SHP as a “Premier Performer” in 2024, ranking among the top 5% of eligible SHP clients for patient satisfaction that year. The agency has also earned a five star rating from the Centers for Medicare and Medicaid Services for patient satisfaction for care provided.
